FIRST PASSPORT:
For first passport, you must have the materials sealed by a USPS employee. Please visit a Regional US Post Office, and take with you an official copy of your birth certificate, including all of the corresponding requirements below. The form must be signed and sealed at the post office and include 2 checks, one to The US Department of State (sealed inside the envelope), and one to PVS International (do not seal in envelope). Then send the envelope to PVS International for expediting. For Minors(Under Age 16) Include: DS-3053 and LOA for Minors1. Fill out DS-11 form (sign & seal at the post office) 2. Attach full copy of DS-11 form to outside of sealed envelope 3. (2) photos 2X2 Inches in size with a light background 4. Official birth certificate 5. Copy of itinerary (if expediting) 6. Letter of authorization (If minor use LOA for minors above) 7. Fees:
RENEWING PASSPORT: If it has been more than 15 years since the issue date on the passport being renewed you must have the materials sealed by a USPS employee. Please visit a Regional US Post Office, and take with you an official copy of your birth certificate, including all of the corresponding requirements below. The form must be signed and sealed at the post office and include 2 checks, one to The US Department of State (sealed inside the envelope), and one to PVS International (do not seal in envelope). Then send the envelope to PVS International for expediting. For Minors(Under Age 16) Include: Minors do not renew passports, they must apply using new passport process. 1. Fill out DS-82 form 2. (2) photos 2X2 Inches in size with a light background 3. Old Passport (will be returned to you) 4. Copy of itinerary (if expediting) 5. Letter of authorization 6. Fees:

MAKE AN AMMENDMENT TO YOUR PASSPORT: For Minors(Under Age 16) Include: LOA for Minors 1. Fill out DS-5504 form (only for less-than 1 year-old passports) 2. (2) photos 2X2 Inches in size with a light background 3. Old Passport 4. Copy of itinerary (if expediting) 5. Letter of authorization (If minor use LOA for minors above) 6. Fees:
LOST/STOLEN PASSPORT: For lost/stolen passport, you must have the materials sealed by a USPS employee. LIMITED/SECOND PASSPORT: 1. Fill out DS-82 form 2. (2) photos 2X2 Inches in size with a light background taken within the last three months. 3. Valid U.S. passport (current passport) 4. Copy of itinerary (if expediting) 5. Letter of authorization 6. 1st Letter: Letter on company letterhead written by the manager or sponsor requesting that a limited passport be issued. Example Manager Letter 7. 2nd Letter: Letter on company letterhead written by passport applicant requesting that a limited passport be issued. Example Individual Letter 8. 3rd Letter: Letter stating that you will notify the State Department if one of your passports becomes lost or stolen. Example Assurance Letter 9. Fees:
